Care plans:The care plans we develop are individually client based and will involve family, main carers from the initial visit.

The care plan is developed following the initial needs assessment completed by one of our managers and information held is in complete confidentiality. Our care plans will basically explain routine tasks to be undertaken by carers, at the specific times agreed.Information will be left for the client at their address and this includes a service user guide (this leaflet), a copy of their care plan including a risk assessment, our contact numbers, our complaints procedure and other useful information. The risk assessment is completed to identify any risks that may occur during a visit, to either the carer or the client.Within the care plan document the carer will write a summary on every visit, which gives a brief overview of any interventions that occurred during the visit. Before leaving, the carer will finally ask the client to authenticate their time sheet.

Regular reviews of care will continue throughout the clients time with Dawnkelly Homesupport.
